The following is required of all members in order to comply with Federal Law and Federation requirements. Any item not checked will not appear in the Club's Yearbook.

Sierra Madre Woman's Club Personal Data Consent

I understand, acknowledge and agree that the items below– where checked– constitutes the giving of my permission to be used, and may be published in the Club's yearbook. I also understand that only those items I check will be published. I understand that it is my duty to inform the Club Membership Chair in writing if my options change.
